Research Unit

The research unit aims to optimize care coordination by nurses for patients with complex needs, particularly during their transition between two different care settings. The unit's work focuses, among other things, on the training of nurses in coordination, the development of a reference framework for this skill, and the analysis of the implementation of innovations aimed at facilitating the transition of people between the emergency department and primary care.

• Integration of health and social services at the systems level.

• The complexity of patients' needs and situations.

• Coordination as a strategy allowing a safe transition between emergency departments and primary care.

• Continuing professional development of nurses in care coordination.

• Interprofessional collaboration and patient partnership.
